Simvastatin Hydroxy Acid, is a potent reversible inhibitor of HMGCR (HMG-CoA reductase). This simvastatin metabolite reduces cholesterol synthesis and increases low-density lipoprotein (LDL) receptors on cell membranes of liver and extrahepatic tissues. Studies suggest that simvastatin hydroxy acid can be determined in plasma using liquid chromatography–UV detection (LC–UV) or gas chromatography–mass spectrometry (GC–MS). Further studies have shown that the cytochromes CYP3A4, CYP3A5, and CYP2C8 all catalyz the formation of simvastatin hydroxy acid.
